Making Tax Digital (MTD) requires doctors with qualifying income to submit income and expense information quarterly through approved software, replacing the traditional annual Self Assessment process for those who are within the scope of MTD.
WHAT?
MTD ITSA is a government initiative by HMRC to transform how landlords and self-employed individuals report their income.
WHY?
The system aims to reduce errors in tax reporting and help taxpayers manage their tax affairs more effectively throughout the year.
WHO?
The requirements apply to self-employed individuals & property landlords. Eligibility is determined by total qualifying income from these sources.
WHEN?
The mandate commenced from April 2026 for those with qualifying income over £50,000. The threshold reduces to £30,000 from April 2027 & to £20,000 from April 2028.
When & How Will Doctors Be Eligible for MTD?
Qualifying income is the total gross professional income doctors receive from all medical work, both in the UK and overseas, before any expenses are deducted.

- MTD for IT applies to individuals with annual self-employment or property income of more than £50,000.
April 2027
- MTD for IT applies to individuals with annual self-employment or property income of more than £30,000.
April 2028
- MTD for IT applies to individuals with annual self-employment or property income of more than £20,000.

Which doctors need to comply with MTD?
You must comply if you are a self-employed doctor, consultant, locum, independent consultant, medical professional with private practice or receive property income, and your gross business/property income exceeds the threshold.
Do salaried doctors, NHS consultants or doctors in partnerships need to register?
If you are salaried only, with no self-employed income and no property business, you may not be in scope.
However:
- Self-employed locum doctors working via agencies or private practice are in scope
- If you receive property income above the threshold, then MTD applies
- Doctors in partnerships may still need to comply for their personal self-employed income streams
What do doctors need to submit under MTD?
Doctors need to submit the following details each quarter:
- Total professional income (consultancy, locum fees, private practice, sessional work)
- Allowable practice-related expenses (rent, insurance, CPD, travel)
- Digital records aligned to HMRC categories. You do not need to submit receipts or invoices each quarter, only totals by category. At year’s end, you submit your final declaration, including other income (dividends, interest, property)
What software do doctors and medical professionals need for MTD?
You must use HMRC-recognised MTD software for doctors (or bridging software) for digital records and submissions. It must support your income streams (self-employment, property) and link to HMRC’s system.
Can my accountant handle MTD for me?
Yes. Your accountant can act as your agent, managing your digital bookkeeping, quarterly submissions and year-end final declaration using MTD software for doctors. However, you remain legally responsible for accuracy and timely filing.
